Had a similar one in my store, but this is definitely a different model.

Controllers like that were made as a response to the initial pad's horrible handling on corners, specifically with Street Fighter in mind. The shoulder buttons were a bit awkward to use attacks with, so they were arranged in a manner similar to the six button Genesis pads, except that it was just a rearrangement rather than added functionality. You don't see too many floating around for the 3DO. In fact, I only ever saw the one.

i have one that looks almost the same but mine has a row of 6 switches at the top instead of the 3 you have

The only one I found that was identical was a photo of a " M2 prototype controller" It is identical but seems to be missing the turbo switches ,etc.

This is actually a 3D Zero Controller. It originally came with a little stick that screws into the d pad. There's a picture of the box and all over on 3DO Today on the accessory page.

I think the stick part is fairly neat although being as small as it is, I can't see it being that useful and likely to break with any type of semi rough usage. Kinda reminds me of those really old joysticks from the 80's, the stick part that is.

The original Sega Master System controllers from 1986 introduced the little joystick to screw into the D-Pad, plus a black plastic cap if you chose to take the joystick off.
